HEALTH RECORDS and IMMUNIZATION REQUIREMENTS
All students must have up-to-date health records, annual physicals, and immunization records on file with the school. These records must be submitted annually before the first day of school and updated annually. Any exemptions must comply with state laws and be documented appropriately.
Please note that all students must have their records updated and submitted before they can attend the school.
Florida-Specific Requirements
- Certification of Immunization (Form DH 680): The Form DH 680, Florida Certification of Immunization, must be used to document receipt of immunizations required for entry and attendance in Florida schools, childcare facilities, and family daycare homes. This form can be obtained from your child’s healthcare provider and must be submitted to the school.
- Required Immunizations:
- Diphtheria-tetanus-acellular pertussis (DTaP)
- Inactivated polio vaccine (IPV)
- Measles-mumps-rubella (MMR)
- Varicella (chickenpox)
- Haemophilus influenzae type b (Hib)
- Pneumococcal conjugate (PCV15/20)
- Hepatitis B (Hep B)
- Tetanus-diphtheria-acellular pertussis (Tdap) for 7th-grade entry
- Exemptions: Medical exemptions must be documented on Form DH 680 and signed by a licensed healthcare provider. Religious exemptions must be documented on Form DH 681, which can be obtained from the local county health department.
- Florida SHOTS™: The Florida State Health Online Tracking System (Florida SHOTS™) is a free, statewide, centralized online immunization registry that helps healthcare providers, schools, and parents keep track of immunization records. Parents can access their child’s certified Form DH 680 with a personal identification number (PIN) issued by the healthcare provider.
